If your student has EVER attended one of our schools you do NOT need to complete a Pre-Enrollment. Please contact your school of enrollment.
Requirements to Register New Students
2026-2027 School Year
Students enrolling in Kindergarten must be five years of age by September 1, 2026
Students enrolling in Transitional Kinder must turn four years of age by September 1, 2026.
1. Verification of Student Date of Birth:
✓ Original Birth Certificate, Passport, Certified Hospital Birth Record, or duly attested Baptismal Record.
2. Verification of Immunizations - Bring the immunization record at the time of enrollment. Your child must have all vaccinations needed. The following immunizations are required for school entry:
✓ 5- DTap, 4- Polio, 3- Hepatitis B, 2- MMR, 2- Varicella.
✓ Tdap is required for students enrolling in the 7th or 8th grade.
✓ Health Examination For School Entry Required.
3. Proof of Residency (ONE of the following is required) - Document must be dated within 30 days:
✓ Gas, Water, Electric, Telephone, rent receipt or a lease agreement. Bill must have parent’s name and date on the document. If unable to provide proof of residence, ask for an Affidavit of residence and return with a proof of address of person who signed the affidavit.
4. Oral Health Assessment:
✓ Only for Transitional Kindergarten and Kindergarten
5. Legal Custody papers – if there are any legal court orders or foster placement papers.
6. Special Education:
✓ If your child has an IEP individualized Education Program
7. Physical Examination (CHDP):
✓ Is required for students enrolling only in the 1st grade. This item is not required for Transitional Kindergarten or Kindergarten.
